Kheir Center provides primary care medical services through multiple clinic locations. Our programs include Primary Care, Women’s Health, Men’s Health, Prenatal Care, Pediatrics, Radiology, Family Planning, Dental, Optometry, and Behavioral Health. For those who qualify, we offer free or low-cost medical care under various County and State programs.

Human Services

Patient Resources Department

 

Kheir Center’s Patient Resources Department offers enrollment in healthcare, social service program and related services for low-income families. With more than 30,000 case visits annually, the Patient Resources Department assists its diverse clientele with issues ranging from eligibility determination for various government assistance programs, including Supplemental Security Income (SSI) and Medi-Cal (Medicaid), to enrollment of children for either Medi-Cal or the Healthy Families Insurance Program. Our culturally-sensitive services encompass assistance with document preparations needed to apply for these programs. Beyond the application and enrollment process, our Case Managers continue their informative services to our clients post-acceptance, ensuring effective utilization of programs for maximum benefits. For clients who may not qualify for subsidized services, we also offer information on alternative free to low-cost healthcare services options.

Adult Day Health Care (ADHC) / Community-Based Adult Services (CBAS)

 

Kheir Center provides daily healthcare for eligible adults with disabilities through the Community-Based Adult Services program at our Adult Day Health Care Centers.

Our services include:
• On-site Care by Registered Nurses
• Personal Assistance: Help with eating, dressing, grooming, and bathroom use
• Physical Therapy: Rehabilitation of damaged muscles
• Speech Therapy: Rehabilitation of basic phonemic skills
• Education and Entertainment
• Hot Meals and Snacks
• Psychiatric and Human Services
• Transportation to and from the ADHC center
• Individual and Group Counseling
• Alzheimer’s Care
• Dementia Care

 

Eligibility
In addition to being at least 18 years of age, a prospective ADHC/CBAS client should meet the criteria as outlined by California’s CBAS program, including:

 

• Diagnosis of a medical condition that requires treatment or rehabilitative services, such as cerebral palsy, autism, epilepsy, Alzheimer’s Disease or other similar developmental disabilities.

 

OR

 

• Diagnosis of a chronic illness, such as stroke, diabetes, hypertension, arthritis, or heart disease.
• Exhibition of a mental and/or physical impairment that impedes daily activities.
• Exhibition of a high potential for further deterioration in the absence of ADHC services.
